New Clients

To open a new account, please:

STEP 1: Fill out our Production Information page.

STEP 2: Sign and date our Rental Agreement. Submit only one Rental Agreement based on your method of payment.

STEP 2b: (Credit Card accounts ONLY): Provide images of the credit card (front and back) and the cardholder’s current driver’s license (front and back).

STEP 3: Obtain a Certificate of Insurance (COI)* that satisfies the following:

• The COI must be current. Expired COIs will not be accepted.
• General Liability: $1,000,000 or more in coverage. Must name Eastern Costume, Inc. as additional insured.
• Property Insurance: Eastern Costume, Inc., named as Loss Payee.

*Orders under $2,500 do not require a COI but may require a deposit equal to the replacement value of the rental item(s) and/or a current and valid credit card to be on file for incidentals. Deposits are refunded via check only, after all rentals are returned and all invoices have been paid.
